In July, FDA's Pharmacy Compounding Advisory Committee (PCAC) recommended six peptides for return to compounding. Since then, the question has been what FDA would actually require.
Now we know.
FDA has published the conditions under which 503A compounding pharmacies may compound and dispense all six: BPC-157, TB-500, KPV, MOTS-c, Epitalon and Semax. Emideltide (DSIP), which PCAC did not recommend, is not included.
This is a real step forward for patient access. It also sets a higher bar than compounders have historically worked under. Here's what it means.
What FDA announced
FDA will exercise enforcement discretion for 503A pharmacies compounding these six peptides, provided they meet a specific set of conditions. Final rulemaking adding the peptides to the 503A Bulks List is still to come. Until then, this policy is the operating framework.
The requirements, one by one
- FDA-published specifications. The bulk drug substance must conform to the exact specifications FDA attached to the policy (Exhibit B). FDA rarely publishes specifications itself; normally manufacturers develop and submit them. FDA appears to want to accelerate supply, but the testing required is more extensive than what's been required historically.
- FDA-registered manufacturers only. Bulk substance may only be purchased from FDA-registered manufacturing facilities. Some manufacturers may be able to retest an existing batch against the new specs and issue a conforming certificate of analysis (COA).
- Adverse event and quality reporting. Serious adverse events and product quality issues must be reported to FDA and the states through MedWatch. 503A pharmacies haven't historically been required to do this and will need to build the procedures.
- Labeling. Product must be labeled as a compounded product, not for resale.
- Single active ingredient. Each product may contain only one API. Popular combinations like the BPC-157 + TB-500 "Wolverine Stack" are not allowed.
- Oral dose forms. Bioavailability requirements make oral forms harder to compound and dispense than injectables.
- Injectable delivery. Delivery systems are limited to what is essentially a standard FDA-cleared syringe.
Compounders must also comply with pharmacy law in the state where the patient lives.
Policy isn't the same as product on the shelf
As we wrote in July, reclassification is not readiness. The same holds now. Most peptide supply on the market today is research-use-only (RUO), and RUO material can't be used for compounding.
Before a pharmacy can dispense, it needs to:
- Source material from an FDA-registered facility that meets FDA's specifications, with a conforming COA
- Put an adverse event reporting program in place
Once both are done, pharmacies can begin compounding batches for patients. That could mean compliant product within a few months.
Where CareValidate stands
We're working with our pharmacy partners now on sourcing and reporting readiness. When compliant supply is available, these six peptides will be added to our Longevity & Wellness category, built on the same provider review, pharmacy fulfillment and compliance infrastructure our storefronts already run on.
What this means for your program
- No stacks. If your roadmap includes combination products, plan for single-ingredient offerings.
- Lead with injectables. Oral forms face a harder path under this policy.
- Expect safety reporting. Patient intake and follow-up will need to support adverse event capture.
- Choose partners carefully. Sourcing, reporting and state compliance now sit with your pharmacy and platform. Ask how they're handling each.
Current offerings: still fully available
Nothing changes for our existing formulary. NAD+, Sermorelin, GHK-Cu, PT-141 and Glutathione remain available across all storefronts, fully LegitScript-certified.
